ERROR - Javascript is disabled

To use the Form A reporting tool Javascript must be enabled.
It has been determined that Javascript is disabled for your browser.
To continue with submitting Form A you will need to enable Javascript.
Please use the insructions below.

To Enable JavaScript in MS Internet Explorer 5.5 or 6 (PC):

• On the menu bar, select "Tools"
• Select "Internet Options"
• Select the tab marked "Security"
• Select the "Internet" zone
• Click on the button marked "Custom Level"
• Scroll down the list, near the bottom, to the category "Scripting" and sub-category "Active Scripting"
• Select the radio button marked "Enable"
  Click "OK", then "Yes", then "OK"

To Enable JavaScript in Netscape 4.7x, 6.x or 7.x (PC or Mac):

• On the menu bar, select "Edit"
• Select "Preferences"
• Select "Advanced"
• Click on the checkbox marked "Enable JavaScript"
• Click on "OK"

To Enable JavaScript in MSInternet Explorer 5.1 for Mac:

• On the menu bar, select "Edit"
• Select "Preferences"
• Under the heading "Web Browser," select the category "Web Content"
• Under the section "Active Content," select the checkbox marked "Enable Scripting"
